Aging Life Care, also known as geriatric care management, is a holistic, client-centered approach to caring for older adults or others facing ongoing health challenges.
First and foremost, our job is to ease your mind. We help you navigate and plan for your loved one's care, so you're not tackling this complicated landscape alone. To do so, we provide a Comprehensive Assessment and Plan of Care. We start by assessing the whole person and their environment by visiting the aging adult in his or her home. We then make recommendations for next steps based on your family goals, considering a spectrum of services in the plan of care, and connect you with benefits and resources tailored to your needs.
After a comprehensive review of your own unique situation, we will propose appropriate and cost-effective services to support you and your loved one, while simultaneously monitoring and evaluating services--making adjustments when needed. One of the more crucial roles we play is that of advocate for the aging adult regarding healthcare matters and assisting with appointments.
