Aging Life Care, also known as geriatric care management, is a holistic, client-centered approach to caring for older adults or others facing ongoing health challenges.
At Aging Well Advocates, we offer options for you to begin
creating a roadmap for your aging loved one based on their situation. Whatever your stage, we can help you prepare
to navigate the challenges of aging and make positive,
informed decisions.
Comprehensive Assessment and Plan of Care
We start by assessing the whole person and their
environment by visiting the aging adult in his or her home.
We then make recommendations for next steps based on
your family goals, considering a spectrum of services in the
plan of care, and connect you with benefits and resources
tailored to your needs.
After a comprehensive review of your own unique situation,
we will propose appropriate and cost-effective services to
support you and your loved one, while simultaneously
monitoring and evaluating services--making adjustments
when needed. One of the more crucial roles we play is that
of advocate for the aging adult regarding healthcare matters
and assisting with appointments.
-Provide ongoing assessments to facilitate care changes
-Help navigate home care
-Coordinate efforts of key support systems
-Establish regular family member communication, near and far
-Navigate state/private insurance regulations
-Referral to legal and financial advisors
-Help elder placement in and advocating for long term care and assisted living facilities
-Assist navigating medical information/ appointments
-Guide aging adult quality of life decisions for families
-Coach families in recognizing signs/ symptoms of memory loss/dementia
-Develop long range plans for aging love ones

We’ll discuss your loved one’s situation and will provide you with a thorough introduction to our services, as well as some direction into the steps to be considered as part of your family's care planning requirements.
