First and foremost, our job is to ease your mind.  We help you navigate and plan, instead of muddling through alone. To get there we  provide a Comprehensive Assessment and Plan of Care. We start by assessing the whole person and environment by visiting the aging adult in his or her home.  From there, we make recommendations for next steps based on your family goals, and consider a spectrum of services in the plan of care, connecting you with benefits and resources tailored to your needs. We can recommend appropriate and cost effective services to support the aging adult and their family. We can monitor and evaluate services while making adjustments when needed. We can advocate for the aging adult regarding healthcare matters and assist with appointments. 


Specific Service Areas Include:

  • Advocate for the aging adult and family caregivers
  • Facilitate effective communication for out of town families
  • Provide ongoing assessments to facilitate changes in care 
  • Help to navigate care provided at home
  • Coordinate the efforts of key support systems
  • Facilitate communication between family members
  • Provide support around aging challenges
  • Navigate state and private insurance regulations
  • Referrals to legal and financial advisors
  • Provide assistance with placement in a long term care or assisted living facility if needed
  • Advocate for the aging adult in a long term care or assisted living facility
  • Navigate medical information and medical appointments 
  • Guide families to decisions that promote quality of life for the aging adult 
  • Assist families in recognizing signs and symptoms of memory loss and dementia
  • Develop long range plans for aging loved ones to subside stress in the future
